On a glorious sunny afternoon, a group of people gathers in a park to discuss happiness.
"I will be happy when I am wealthy," one says
"My happiness completes when I am when I get my promotion at work," says another.
The religious guy says "when I die and all suffering ends that's happiness"
We are all happy now at this moment in the glorious sun and the beautiful park some agree.
An old man sitting across a park bench overhears this quip
"If I can be healthy and live a full life that is when I will be happy";
For centuries we have searched for the measurement and the meaning of this feeling in humans
In the context of mental or emotional states, happiness could be defined as a pleasant or positive experience.
I think the old man is correct.
Without health, everything seems meaningless
When we are not well Primary Care is the first step
The journey begins when a patient (or client) enters the health system and continues through each point of care toward diagnosis

Proven home remedies first and then, visit a General Practitioner (GP)and then exit care.
The journey could be single have multiple stages or be ongoing.
Ideally, after one or two visits, a dose of medicine will cure the illness.
While this is true in most cases, some patients require continual care due to the ailments progressing to a chronic disease stage.

We all like to be healthy and live a full life without being sick or having to visit a doctor. Unfortunately, the fact of life is that human beings develop health issues over their lifetime.

• exercising regularly,
• limiting alcohol intake
• not smoking
What is Chronic Disease?
A chronic disease is a medical condition or illness that is long-lasting or recurrent. It includes conditions like
arthritis, asthma, back pain, cancer, chronic obstructive pulmonary disease, diabetes, and mental health conditions.
Living with Chronic Disease
Living with a chronic condition means adjusting life to accommodate the condition and living with the demands of the illness and therapy used to treat the condition.
Chronic conditions may change the way a person lives, sees themselves, and how relates to others. Living with the condition may add a great deal of stress to a person's life.
What is a care plan
For health issues lasting for six months and more there are two Medicare-funded plans
• General practice Management Plan (GPMP)
• Team Care Arrangements (TCA)
While GPMP involves a doctor and patient care relationship the TCA is supported by a team of allied health services provided to assist and treat the patient.
THE PLAN
An individually tailored step-by-step process of monitoring and checking by the doctor and recording so that early signs are detected and treated without delay ensuring ongoing and long-term health and well-being.
It is essential for people with chronic diseases or conditions.
The doctor and nurse will discuss all the details for participating patients.
Although approximately half of all Australians live with a chronic condition, Australians are considered to have one of the longest life expediencies in the world.
Chronic diseases are the leading cause of illness, disability, and death in Australia.
Chronic diseases are also referred to as chronic conditions, chronic illness, and non-communicable conditions.
What can be done?
People plan their finances, retirement, study, holidays building houses, and even a birthday party.

To live with a chronic condition or even find a cure, Australian Medicare together with General practitioners are encouraging patients to participate in a fully funded health care system usually referred to as a care plan.
